These lower-than-expected rates risked both missed incentive payouts and lower star ratings. Compounding the issue was the underutilization of valuable CCD data from large provider groups that could fill these gaps and improve reporting accuracy.

SOLUTION SUMMARY

Data Ideology worked with the client to unlock the potential of CCD data by transforming it into a usable format. CCDs from a large provider group were mapped from XML and processed through ETL workflows into the client’s Snowflake instance. The data was then reformatted to match the existing EMR structure and fully integrated into the client's HEDIS reporting solution—allowing for immediate inclusion in measure calculations and downstream analytics.

  1. The client had access to Continuity of Care Documents (CCDs) that weren’t yet part of its centralized data infrastructure.
